Tattoo Numbing Cream: What Works and What Doesn’t

Numbing creams have gotten better in the last decade. They’re no longer the gimmick they used to be. But they’re not magic either — and some actually interfere with tattooing. Here’s an honest guide.

What Numbing Creams Actually Do

Most numbing creams contain lidocaine — a topical anesthetic that blocks pain signals from skin nerves. Some also include:

  • Prilocaine or tetracaine — additional anesthetics
  • Epinephrine — constricts blood vessels (reduces bleeding)
  • Benzocaine — alternative anesthetic

Applied to intact skin 30-60 minutes before the session, the cream penetrates into the dermis and numbs the area for 1-3 hours.

Creams That Actually Work

  • Zensa (lidocaine 5%) — widely used, solid effect
  • BLT cream (benzocaine + lidocaine + tetracaine) — prescription version, strongest
  • Hush Numbing Cream — tattoo-industry-specific, lidocaine 5%
  • Dr. Numb — lidocaine 5%, very popular
  • Emla — pharmacy standard, lidocaine + prilocaine

All work best on high-pain spots: ribs, sternum, inner arm, armpit, feet, hands. See our pain chart.

Why Some Artists Don’t Like Numbing Cream

Honest warnings from the artist’s perspective:

  • Can change skin texture — some creams make skin slightly rubbery or swollen, changing how the needle feels
  • Wears off mid-session — the numbing typically lasts 60-90 minutes. A 4-hour session will have hours of normal pain
  • Can cause uneven healing — some epinephrine-containing creams affect blood flow, affecting ink uptake
  • False confidence — clients sometimes get bigger tattoos than they can actually tolerate, then struggle when the cream wears off

Always ask your artist before using numbing cream. Some prefer you skip it; others are fine with it; some have preferred brands.

Best Use Cases

  • Very painful placements — ribs, sternum, armpit, inner bicep, feet, hands
  • First tattoo — if nervous, it helps with initial anxiety
  • Short sessions — under 90 minutes where cream stays effective the whole time
  • Touch-ups on old tattoos — particularly uncomfortable areas

Skip Numbing Cream When

  • Low-pain placements — outer upper arm, outer thigh, calves — cream is unnecessary
  • Long sessions (4+ hours) — it’ll wear off before you’re done anyway
  • Your artist prefers you don’t — respect their preference
  • First-time buyer of a random brand — stick with tested brands

How to Use It (If You Do)

  • Apply 30-60 min before session (check product instructions)
  • Use a thick layer
  • Cover with plastic wrap to improve absorption
  • Clean thoroughly before session
  • Tell your artist you used it

The Real Truth About Tattoo Pain

Most tattoo pain is temporary and manageable without cream. Adrenaline kicks in within 10-15 minutes. Breathing helps. Breaks help. Eating well beforehand helps. See our first tattoo tips.

Don’t let fear of pain dictate where or how you get tattooed. A tattoo you wanted on your ribs but got on your forearm because you were scared of the pain is a tattoo you don’t actually have.
